Two cases of subacute combined degeneration: magnetic resonance findings.

چکیده
We report two cases of subacute combined degeneration. Both patients had undergone total gastrectomy. The chief complaints were numbness in both upper extremities in case 1 and numbness in both the upper and lower extremities and gait disturbance in case 2. The pain, temperature, and vibration senses of both patients were decreased. Laboratory examinations showed macrocytic anemia and a decreased serum vitamin B12 level in both cases. In both cases T2-weighted magnetic resonance images showed an area of hyperintensity in the dorsal columns of the cervical spinal cord. The patients were treated with vitamin B12. The abnormal signals had disappeared on follow-up magnetic resonance examination 1 year later in case 1 and 3 months later in case 2. These patients showed neurological improvement, but the numbness in the upper extremities persisted even after the area of abnormal signal intensity had disappeared in case 1.

Subacute combined degeneration ( SCD) due to vitamin B12 deficiency is common in North of Europe but sometimes it is seen in Iran. It can develop before the symptoms of anemia is obvious, with myelin injury and then axon and even neuron destruction in spinal cord, brain, optic and peripheral nerves. In this study two cases are reported; in the first patient, this disease in manifested by vitami...

INTRODUCTION Subacute combined degeneration of the spinal cord is a rare cause of demyelination of the dorsal and lateral columns of the spinal cord and is a neurological complication of vitamin B12 deficiency. Subacute combined degeneration without anemia or macrocytosis is rare. CASE PRESENTATION We present a case of cobalamin deficiency in a 29-year-old Moroccan woman who presented with su...

Deficiency of Vitamin B12 can have many neurologic sequelae including periperhal neuropathy, visual disturbances, mood and memory changes and subacute combined degeneration (SCD) of the spinal cord1. Macrocytic anemia is also a common finding of B12 deficiency allowing the clinician to arrive at a diagnosis, however neurologic symptoms can be present even in the absence of hematologic changes. ...
